Case Management Services Greccio Housing partnered with Harbor House Collaborative to wrap care coordination around chronically homeless individuals with a disability who are in housing units at a local apartment community in Colorado Springs. The Case Manager assists the clients with creating specific goals and accessing services within El Paso County which address the needs of the "whole person" with the ultimate goal of self-sufficiency and integration back into the community.
